On behalf of the Medford Surgery Center, LCC, we are pleased to notify you and the public that beginning today, September 30, 2020, $474,937.80 is being offered to the community to fund Community Health Initiatives (CHI).
This funding is available per the Massachusetts Department of Public Health (DPH) Determination of Need (DoN) Regulation, in association with DoN Application #18060613-AS, for the Medford Surgery Center, LCC. (a joint venture of MelroseWakefield Healthcare).
This CHI aligns with the Massachusetts Department of Public Health goals of identifying, understanding, and addressing the underlying root causes of health disparities and promoting health equity through policy, system, and environmental change. This CHI will focus on addressing behavioral health, specifically, mental illness/mental health, and the social determinations of health (SDoH) impacting at-risk individuals and the community.
The RFP is due on November 30, 2020, at 5:00 pm; further instructions are included in the RFP. Bidders Conferences will be offered remotely in October. Please share with any community agencies, partners, groups or organizations that are appropriate.
We held a virtual informative bidders conference, on October 22 and 27. We gave an overview of our Community Health Initiative (CHI) RFP, offered technical assistance and answered any questions you may have regarding the RFP.
