Accessing your Medical Records
You have a right to access your medical records and reports.
Being an active participant in your healthcare empowers you to make the best health decisions. Your medical records may contain:
- imaging results
- immunization records
- lab results
- physician notes from medical visits
We make it easy to access your medical records. You can request that your records are sent to a physician’s office, to receive copies or to view on an app.
You can also access your records through our patient portals.
How to Access
The Federal Privacy Law (HIPAA) enacted on 4/14/03 requires certain elements to be contained on all authorizations for disclosure of protected health information (your medical records).
Requesting Your Forms are Sent to a Physician’s Office
You need to complete the Authorization to Use & Disclose Protected Health Information form to send files to a physician office. Requesting Copies of Your Medical Records
You need to complete the Authorization to Use & Disclose Protected Health Information form to receive paper copies or radiology images on a CD for personal use. Access from an App
For personal use, you can view your hospital records from your smartphone by downloading an app. To start, you’ll need to download the health management app of your choice (such as 1upHealth). Once you choose an app, you’ll need to follow the app directions and then complete and return a medical release form to us so we can release your records.
App features and functionality vary, but you may be able to maintain a medication list and view your real-time patient data from a recent hospital stay, lab results, medications, imaging reports, etc. If you have questions about the app, we encourage you to ask the app developer.
It’s important to note that the information in your app is private and will not alter your hospital medical record, please update your doctor about any changes in your health.
