Accessing your Medical Records

You have a right to access your medical records and reports.

Being an active participant in your healthcare empowers you to make the best health decisions. Your medical records may contain:

  • allergies
  • imaging results
  • immunization records
  • lab results
  • medications
  • physician notes from medical visits

We make it easy to access your medical records. You can request that your records are sent to a physician’s office, to receive copies or to view on an app.

You can also access your records through our patient portals.

How to Request and Access Your Medical Records

The Federal Privacy Law (HIPAA) enacted on 4/14/03 requires certain elements to be contained on all authorizations for disclosure of protected health information (your medical records).

You need to complete the Authorization to Use & Disclose Protected Health Information form to send files to a physician’s office. This request may be made in-person at the medical records office at MelroseWakefield Hospital, at your physician’s office, or faxed to 781-979-3217 with a copy of a picture ID.

If you have any questions, please contact our medical records department at 781-979-3213.

View Your Medical Records on an App

For personal use, you can view your hospital records from your smartphone by downloading an app. To start, you’ll need to download the health management app of your choice (such as 1upHealth). Once you choose an app, you’ll need to follow the app directions and then complete and return a medical release form to us so we can release your records.

App features and functionality vary, but you may be able to maintain a medication list and view your real-time patient data from a recent hospital stay, lab results, medications, imaging reports, etc. If you have questions about the app, we encourage you to ask the app developer.

It’s important to note that the information in your app is private and will not alter your hospital medical record, please update your doctor about any changes in your health.

If you have questions about your medical records, how to connect your medical records to the app or need to report any errors, please contact our medical records/API helpline at 781-338-7200 and select option #3.
