We offer two patient portals – one for your hospital stay and radiology visits and one for your physicians’ office visits.

For questions or issues accessing either Patient Portal, please call 781-338-7200 on Monday through Friday, 8 a.m. to 4 p.m.

Physician Office

You can connect with your physician’s office through this secure portal. Through this portal you can:

  • refill prescription
  • request an appointment
  • send a message to your primary care physician or specialists (including OB/GYN, cardiologist, endocrinologist, and more)
  • view your health information (including allergies, advanced directives, health conditions, medication list, immunizations and routine lab results)

You will need a PIN number to access this portal. Contact your physician’s office to receive your unique PIN.

PHYSICIAN OFFICE PATIENT PORTAL

Hospital Stays & Appointments

The hospital-based patient portal provides secure access to your electronic medical record. View your laboratory results, imaging results (x-ray, CT, MRI), medications, allergies, medical problems, discharge instructions, visits, appointments and more.

Use this portal if you have received any of these services:

You will need a PIN number to access this portal. If you need a PIN, you can request one in-person at the Medical Records department at MelroseWakefield Hospital (Monday-Friday from 8 a.m.-4 p.m.) or by completing and faxing the Authorization to Use & Disclose Protected Health Information form with a copy of a photo ID to 781-979-3217.
