Advanced Medical Directives
A health care proxy should be established so that your “health care agent” can make medical decisions for you if you become incapable of making your own medical decisions (e.g. due to unconsciousness, dementia or other mental limitations). You can appoint almost any adult over the age of 18 to be your agent. An agent can be a family member, your spouse, a same-sex or domestic partner, an adult child, a friend, or someone you trust.
